To request Academic Accommodations:
- Submit a Student Accessibility Accommodation Request Form
- Submit relevant and comprehensive documentation
- Log into the Assumption Accommodate System> to schedule an intake appointment using the Assumption Accommodate system and fill out the Student Intake Questionnaire
- Meet with SAS staff to complete the intake process
