To request Dining Accommodations:
- Submit a Student Accessibility Accommodation Request Form
- Log into the Assumption Accommodate System to download the Physician Support Form located in the Resource Library
- Upload the Physician Support form into the Accommodate System
- A staff member will contact you to discuss next steps
